How do I know if I'm eligible?​
When you initially enroll, you'll input information that will determine your eligibility. Some of the information we need to determine eligibility includes body weight, height, ethnicity, and more. If you are not eligible for some reason based on the criteria, you'll receive a message informing you of the ineligibility before the enrollment process is complete.

How do I enroll?
Enrolling is simple; just click here. The enrollment site will walk you through the steps to provide the information needed. Upon successful enrollment, you'll receive 2 emails at the email address you provided which will give further direction.

How much does it cost?
The cost to participate in IMPACT Diabetes is normally $525 for the year. Right now, until our sponsored slots are full, you can sign up for this program for free through our CDC-supported cooperative agreement. You can also talk with your health insurance company to see if your participation is a covered benefit within your health plan.​

What if something unexpected comes up and I can't finish the program?
IMPACT Dia​betes is structured to be flexible with your schedule. If you miss a few classes, you can complete them at another time with the "Make-up class" feature. You can also request to switch your in-person or telehealth class if your work or family schedule has changed. If you still are unable to participate, you can be withdrawn from your current cohort and placed on a waitlist to possibly rejoin another IMPACT Diabetes cohort in the future.

Who will my information be shared with? Is this secure and private?
IMPACT Diabetes will only share your personal information with coaches and administrators on a need-to-know basis. Your de-identified data, such as weight change and minutes of physical activity will be shared with program sponsors without any identifiable personal information included.
